Feedback & Reporting

SHSU-COM provides confidential and anonymous ways for students, faculty, staff, and other stakeholders to share feedback or report concerns.

Students, faculty, staff, and external stakeholders are encouraged to use SHSU-COM’s Feedback Form to confidentially submit input or concerns. The form is reviewed by the Senior Associate Dean for Educational Affairs and routed to the appropriate leader when needed.

Please provide as much detail as possible to allow for effective follow-up.

Note: Course and faculty-related feedback should be submitted via end-of-course evaluations, not through this form.

The feedback form offers an anonymous method for stakeholders to provide feedback directly to SHSU-COM leadership.

Once submitted, entries are reviewed and assigned for resolution by SHSU-COM leadership. To maintain transparency, responses to feedback are posted publicly when appropriate.

Title IX and Grievance Reporting

Sexual misconduct, Title IX concerns, and grievances are managed through SHSU’s Student Experience & Retention office. Services include medical care, counseling, legal support, and law enforcement contact.

Students may also contact the SHSU Dean of Students or the Commission on Osteopathic College Accreditation (COCA) to report violations of accreditation standards.
